Abstract

Introdução: A Leucemia mielóide crônica atípica (LMCa) é uma neoplasia hematológica que faz parte do grupo de doenças classificadas pela OMS (2008) como Síndromes mielodisplásicas e mieloproliferativas (MDS/MPN). É denominada atípica pela ausência do cromossomo Philadephia, resultado da fusão gênica BCR-ABL1, critério obrigatório para leucemia mielóide crônica. Objetivo: Abordar sobre a enfermidade LMCa, suas características, critérios diagnósticos, citogenética, e sobretudo, em alvos moleculares estabelecidos e em estudo. Métodos: Revisão Integrativa de Literatura realizada através de levantamento bibliográfico, nas plataformas PubMed, LILACS, Scielo e MedLine. Foram aplicados critérios de inclusão como publicação entre 2009 a 2019, enfoque na LMCa, e critérios de exclusão como enfoque na LMC típica, trabalhos com discussão sobre a classificação da OMS (2008), entre outros. Logo, foram selecionados 21 artigos utilizados para compor este presente trabalho.  Resultados: Ainda não há consenso sobre qual mutação desencadeia a patologia da doença, sendo indicadas as mutações ASXL1, SETBP1, SRSF2, TET2 e NRAS como as principais relacionadas à neoplasia. Devido à ausência de achado patognomônico, o diagnóstico é estabelecido através de critérios diagnósticos clínicos e laboratoriais obrigatórios postulados em 2016 pela OMS. Sobre o tratamento, o cenário permanece obscuro, sendo que o único tratamento definitivo é o transplante de medula óssea.  Conclusões: A LMCa é caracterizada principalmente por alta contagem de leucócitos, organomegalia, anemia e falência da medula óssea. As perspectivas futuras dependem ainda do melhor esclarecimento da fisiopatologia da doença e da mutação desencadeadora do processo neoplásico, para que assim, sejam estabelecidos alvos moleculares terapêuticos, resultando no melhor prognóstico da doença.
