O uso da cirurgia robótica e da videotoracoscopia no tratamento do câncer de pulmão: uma revisão sistemática / The use of robotic surgery and videothoracoscopy in the treatment of lung cancer: a systematic review

Abstract


Introdução: O câncer de pulmão é a neoplasia com maior número de mortes anualmente. Assim, recentemente, implementaram-se novas intervenções como a cirurgia torácica vídeo assistida e a cirurgia robótica. Objetivo:  Apresentar as diferenças entre os desfechos da VATS e da RATS para o tratamento de câncer de pulmão. Metódos: Artigos publicados entre 2016 e 2021, incluindo estudos originais, ensaios clínicos, estudos retrospectivos e caso-controle foram selecionados. As bases de dados Medline, SciELO, LILACS, Ovid e Cochrane foram consultadas na busca. Foram encontrados 32 artigos e, após aplicação dos critérios de inclusão e exclusão, 12 estudos foram lidos na íntegra; 4 foram elegíveis para essa revisão sistemática. Resultados e discussão: O tamanho da amostra dos artigos incluídos na revisão variou de 10 a 470 pacientes, sendo a faixa etária  entre 55 e 68 anos em todos os estudos analisados retrospectivamente. Os seguintes desfechos foram comparados: complicações perioperatórias, tempo de duração de internação hospitalar, tempo de duração da cirurgia, taxa de conversão e taxa de mortalidade. A única diferença significativa encontrada foi uma menor taxa de conversão na RATS, por vantagens técnicas. Conclusão: Apesar da menor taxa de conversão na RATS, em geral, os desfechos não apresentaram diferença significativa.
