Transposição do nervo alveolar inferior para o tratamento de hiperalgesia mecânica em mandíbula atrófica / Treatment of mechanical hyperalgesia in atrophic jaw by inferior alveolar nerve transposition

Abstract


Introdução: Descrever o tratamento cirúrgico de um caso incomum de hiperalgesia mecânica associada à compressão dos nervos alveolar inferior e mentual em uma paciente edêntula usuária de prótese total, com mandíbula atrófica. Relato de caso: Este parece ser o primeiro estudo a descrever a transposição dos nervos alveolar inferior e mentual como tratamento da hiperalgesia mecânica em paciente com mandíbula atrófica, usuária de prótese total removível. As estruturas nervosas associadas foram deslocadas, cirurgicamente, para uma posição mais posterior e vestibular, fora da área chapeável da prótese. Apesar de não ter havido lesão aparente de estruturas nervosas, a laserterapia de baixa intensidade associada ao composto citidina monofosfato + uridina trifosfato + hidroxocobalamina foram indicados. Considerações finais: A cirurgia de transposição dos nervos alveolar inferior e mentual pode ser indicada para o tratamento da hiperalgesia mecânica em pacientes edêntulos com atrofia mandibular severa. O procedimento cirúrgico e as terapias complementares utilizadas para recuperação das funções neurossensoriais e neuromotoras apresentaram prognóstico favorável.
