Abstract

Introdução: Os coronavírus (CoVs) são um grupo de vírus de RNA que podem causar infecções respiratórias e gastrointestinais. A maioria dos pacientes infectados é assintomática ou apresenta sintomas leves a intermediários, podendo evoluir para insuficiência respiratória grave e óbito em uma parcela desta população. Muitos pacientes têm apresentado, também, sintomas neurológicos, o que evidencia a necessidade de elucidar a relação patogênica entre a infecção por SARS-CoV 2 e o Sistema Nervoso Central (SNC). Métodos: Foi realizada uma revisão de literatura nas bases de dados Science Direct, Pubmed e Cochrane Library, com os descritores Coronavirus, COVID-19, Neurological e Central Nervous System (CNS). Resultados: 52 artigos foram encontrados, sendo 37 excluídos por serem duplicados ou não se adequarem ao tema, resultando nos 15 elegidos para a revisão. Discussão: Tendo em vista os recentes relatos de sintomas neurológicos em pacientes infectados pelo vírus da COVID-19, foram elaborados três pilares fisiopatológicos à luz da literatura para explicar a virulência neural. A primeira é relacionada aos receptores da Enzima conversora de angiotensina-2 (ECA2) no endotélio vascular cerebral. A segunda é referente à neuroinvasão via neuronal trans-sináptica, por meio dos nervos olfativos. E a última, elucida uma resposta inflamatória reacional com quebra da barreira hematoencefálica. Um estudo em Wuhan (China) observou que sintomas neurológicos eram encontrados em 36,4% dos pacientes com COVID-19 e 45,5% dos casos graves. Dentre eles, tontura, cefaleia, alteração de consciência, convulsões, ataxia e eventos cerebrovasculares agudos, foram os mais relatados. Além disso, também foram relatados casos de encefalite viral, meningite, encefalopatia necrosante hemorrágica aguda e síndrome de Miller Fisher. Conclusão: Tendo em vista os recentes relatos de acometimento neurológico pelo SARS-CoV 2, seus impactos na saúde e qualidade de vida dos pacientes e a escassez estudos na área, é necessário mais estudos para elucidar os meandros clínicos que permeiam os pacientes com tais comorbidades.
