Abstract

As convulsões que ocorrem durante o período perioperatório podem ser causadas por distúrbios metabólicos, abstinência de drogas e álcool ou cirurgia intracraniana, ou raramente podem estar relacionadas à anestesia. Os principais problemas perioperatórios em pacientes com doença de Parkinson são a deglutição e a função pulmonar prejudicadas, podendo levar à pneumonia. O uso de meperidina, para analgesia pós-operatória, deve ser evitado em pacientes em uso de inibidores da monoamina oxidase tipo B, selegilina, rasagilina. Os pacientes também podem apresentar risco aumentado de delírio pós-operatório. O regime de medicação deve ser interrompido o menos possível e os medicamentos que podem exacerbar a miastenia devem ser evitados.  Para pacientes com distúrbios neuromusculares desnervantes e distúrbios neurológicos associados à fraqueza muscular, o agente bloqueador neuromuscular despolarizante succinilcolina deve ser evitado durante a anestesia devido ao risco aumentado de hipercalemia grave. Esses pacientes podem ter uma resposta variável a NMBAs não despolarizantes, como rocurônio, vecurônio, cisatracúrio.
