Angioplastia versus cirurgia de revascularização miocárdica no tratamento da doença da artéria coronária esquerda: uma revisão de literatura / Angioplasty versus myocardial revascularization in the treatment of the left main coronary artery disease: a literature review

Abstract


INTRODUÇÃO: A doença arterial coronariana ocorre pelo acúmulo de placas ateroscleróticas nas artérias cardíacas. A intervenção coronária percutânea (ICP) e a cirurgia de revascularização do miocárdio (CRM) são técnicas utilizadas para a revascularização em pacientes com doença de tronco de coronária esquerda. METODOLOGIA: Trata-se de uma revisão literária mediante análise de artigos científicos da base de dados PubMed com os descritores “angioplasty”, “left main coronary disease”, “myocardial revascularization” e “survival”, incluindo ensaio clínico, metanálise, ensaio randomizado controlado e revisão sistemática, com publicações dos últimos 5 anos com texto completo. RESULTADOS: Após a aplicação de critérios de inclusão e exclusão foram selecionados 15 artigos para a amostra final da revisão. DISCUSSÃO: De modo geral, os pacientes tratados com a CRM obtiveram resultados superiores aos submetidos à ICP, mesmo quando esta empregou técnicas avançadas, como o uso dos stents farmacológicos. Todavia, ressalta-se que, em certos aspectos, a ICP também se mostrou uma estratégia terapêutica eficaz, pois, além de ser menos invasiva que a CRM, demonstrou equivalência a esta no que se refere à incidência de desfechos primários cerebrovasculares. CONCLUSÃO: A CRM é considerada o tratamento padrão para a doença arterial em tronco coronariano esquerdo, apesar da melhora da eficácia da ICP com stents farmacológicos.
