Abstract

  O tumor fibroso solitário da pleura (TFSP), também chamado de mesotelioma, refere-se a um tumor mesenquimal raro que é originado na pleura. Os TFSPs localizam-se em 80% na pleura visceral e em 20% na parietal. Identificar a efetividade do tratamento cirúrgico em casos de tumores fibrosos solitários da pleura. Realizou-se uma revisão de escopo com o checklist PRISMA, utilizando a seguinte pergunta de pesquisa (estratégia PICO): “Qual a efetividade da cirurgia torácica no tratamento de um tumor fibroso solitário de pleura?”. A pesquisa foi realizada em março de 2021 pelas plataformas: National Library of Medicine (MEDLINE) e Scientific Eletronic Library Online (SCIELO), buscando artigos publicados entre 2016 e 2021. Os TFSP podem apresentar características benignas e malignas, sendo a primeira representando 80% dos casos e os TFSP malignos (TFSPm) 20%. Os critérios de malignidade são: crescimento invasivo com limite obscuro, alta densidade celular, 4 ou mais mitoses por 10 campos de visão de alta ampliação, pleomorfismo, presença de sangramento e necrose. A apresentação clínica desses tumores é variável. Geralmente, os TFSPs são identificados em radiografias de rotina. A ressecção cirúrgica completa para todos os TFSPs benignos e malignos é a base do tratamento, além de ser considerada o melhor preditor de sobrevida para todos os tumores fibrosos pleurais. A excisão cirúrgica completa costuma ser curativa em todos os casos benignos e em metade dos casos malignos. O tratamento cirúrgico em casos de TFSPs é considerado o padrão-ouro, além de garantir melhor preditivo de sobrevida. Ademais, vale salientar a necessidade de acompanhamento multiprofissional e planejado, mesmo após o procedimento cirúrgico para avaliação de uma terapia adicional.
