Abstract

A hemorragia pós-parto (HPP) é uma condição potencialmente fatal, sendo a segunda causa de morte materna no Brasil. Define-se por perda sanguínea superior a 500 ml após partos vaginais e 1000 ml após cesarianas, tendo como principais causas: atonia uterina, trauma do canal vaginal, retenção placentária e coagulopatias. O objetivo desse estudo foi identificar as principais medidas de prevenção e manejo da HPP preconizadas pela comunidade científica. A literatura das bases de dados LILACS e MEDLINE foram revisadas. Utilizou-se como termos de busca "hemorragia pós-parto" e "prevenção e manejo da hemorragia pós-parto", o operador booleano "AND" foi aplicado para o cruzamento de dados. Nessa análise incluiu-se revisões sistemáticas e meta-análises publicadas entre 2015 e 2020 nos idiomas inglês e português, desde que o texto completo estivesse disponível. Encontrou-se 77 artigos, destes 40 selecionados para esta revisão. O manejo ativo do terceiro estágio do trabalho de parto reduz o risco da HPP. Diante disso, medidas interventivas como adição de uterotônicos, a exemplo da ocitocina, pinçamento tardio do cordão, tração controlada do cordão umbilical e massagem uterina, são práticas recomendadas para uso rotineiro. Estudos recentes avaliam a possibilidade de substituição da ocitocina profilática por carbetocina. Esses estudos garantem meia-vida e ação uterotônica superior para a carbetocina, embora tenha custo vultoso e indisponibilidade no Brasil. Uma vez que as medidas preventivas não foram suficientes, o reconhecimento precoce da causa da HPP é impreterível. De modo geral, a HPP é diagnosticada por instabilidade hemodinâmica da parturiente. Quando confirmada, utiliza-se uterotônicos, como ocitocina, metilergonovina e misoprostol. Nesta sequência, sendo que o medicamento seguinte deve ser introduzido à terapia quando não houver resposta ao anterior. Existe orientação para o uso simultâneo desses medicamentos. Fica a critério do profissional a conduta a ser abordada. Além disso, há evidências de que o uso precoce do ácido tranexâmico reduz o tempo de sangramento. Em casos de falha na terapia medicamentosa, condutas cirúrgicas devem ser adotadas, preservando, sempre que possível, a fertilidade da parturiente. Inicia-se com as suturas compressivas. A embolização arterial é considerada como tratamento de segunda linha na HPP refratária à terapia inicial, cujo acesso é pouco disponível. A histerectomia parcial é aplicada quando os demais métodos falharam. Conclui-se que a alta taxa de mortalidade materna causada por HPP, requer medidas efetivas que visem minimizar esse desfecho, de modo que todo serviço de assistência ao parto precisa conhecer e praticar o correto manejo dessa condição.
