Associação de marcadores nutricionais com variáveis clínicas, dose de diálise e inflamatória em pacientes tratados por hemodiálise / Association of nutritional markers with dose of dialysis and inflammatory clinical variables in patients treated by hemodialysis

Abstract


A desnutrição, inflamação e aterosclerose, podem justificar grande parte das altas taxa de mortalidade dos pacientes em Hemodiálise. A desnutrição é uma condição comum nesta população e está associada a perda de peso, depleção de tecido adiposo e muscular, assim como baixos níveis de proteínas séricas. Com o objetivo de avaliar a associação das ferramentas de avaliação do estado nutricional com variáveis demográficas, clínicas, laboratoriais e inflamatória em pacientes com doença renal crônica tratados por hemodiálise, foram estudados 82 pacientes em hemodiálise regular, submetidos a uma única avaliação composta de parâmetros nutricionais (Antropometria e avaliação subjetiva global (ASG)) além de dados clínicos, demográficos e laboratoriais. Para verificar possíveis associações entre as variáveis clínicas, laboratoriais e inflamatória com a presença de alterações dos parâmetros nutricionais e desnutrição, avaliada pela ASG, foi realizada análise univariada e posteriormente foi construído modelos de regressão logística para avaliar a presença de associações independentes. A média de idade foi de 60± 13,45 anos, sendo 64,6 % sexo masculino, vinte e nove pacientes (35,3%) eram desnutridos pela classificação da ASG e trinta e três (40,2%) pacientes foram considerados desnutridos pelo IMC. Maiores valores séricos de proteína C reativa se associou com menores valores de circunferência muscular do braço(CMB), pontuação da ASG e nível sérico de albumina. Em relação aos desnutridos avaliados pelo IMC se associaram independentemente com a menores valores de CMB, creatinina e menor presença de sexo masculino. A ASG não teve associação significativa com nenhum parâmetro estudado.
