Incidência de Trombose de Stent Após Intervenção Coronária Percutânea no Infarto Agudo do Miocárdio / Incidence of Stent Thrombosis after Percutaneous Coronary Intervention in Acute Myocardial Infarction

Abstract


Objetivo: Neste estudo, buscou-se averiguar a incidência e avaliar os preditores, as comorbidades mais frequentes e o prognóstico da TS, comparando seus subtipos. Revisão Bibliográfica: A incidência de TS variou de 1% a 6,9%, com valores entre 0,39% e 4,54%, para precoce; 0,59% a 3,9%, para tardia; e 0,52% a 2,28%, para muito tardia. A incidência relacionada aos tipos de stent foi semelhante, apesar de pouco maior para o metálico, relacionado inicialmente a maior ocorrência de reestenose intra-stent, o que diminuiu com o uso do stent farmacológico, observando-se, contudo, maiores taxas de trombose tardia e muito tardia. Descontinuação precoce da terapia antiplaquetária e diâmetro do stent foram alguns dos preditores mais citados e obesidade, diabetes, hipertensão, tabagismo, e idade, os fatores de risco. A maioria dos estudos apontou maior risco de reinfarto, mortalidade em curto e longo prazo, e desfechos desfavoráveis. Associou-se maior risco à presença de comorbidades, IAM prévio e menor taxa de filtração glomerular. Considerações Finais: A TS demonstrou ser uma complicação rara, porém de extrema relevância clínica. Vários fatores influenciam sua ocorrência, fazendo-se essencial a avaliação e o acompanhamento individual e longitudinal.
