Abstract

O transtorno de compulsão alimentar periódica (TCAP) teve seu diagnóstico definido na formulação do V Manual Diagnóstico e Estatístico de Transtornos Mentais (DSM-5) em 2013. Contudo, ainda não há especificações referentes aos aspectos neurocognitivos do transtorno, fato que contribui para casos de comorbidade diagnóstica, por exemplo, com a obesidade. Sendo assim, o presente estudo de revisão de literatura teve como objetivo sintetizar os dados existentes sobre os mecanismos neurobiológicos que caracterizam o transtorno de compulsão alimentar periódica. A pesquisa bibliográfica foi realizada em março de 2019 nas bases de dados PubMed/MEDLINE com descritores "binge eating disorder" e "neuroimaging". Essa busca encontrou 69 artigos no PubMed, dos quais 52 foram excluídos pelo título e 5 pela leitura do resumo, e 44 artigos no MEDLINE, dos quais 38 foram excluídos pelo título e 5 por já terem sido selecionados na busca pelo PubMed; totalizando uma amostra com 13 artigos como amostra. Os estudos constataram alterações na ativação de determinadas regiões cerebrais-ínsula, córtex orbifrontal (OFC), córtex pré-frontal medial (mPFC), córtex cingulado anterior (ACC), amígdala e estriado ventral. Portanto, o não estabelecimento das bases neurais características do transtorno de compulsão alimentar periódica demonstra uma lacuna no âmbito clínico, ressaltando a necessidade de uma investigação neurobiológica detalhada a fim da obtenção de um diagnóstico mais preciso.
